Privacy Policy

Section 1

What This Website Collects

This website does not collect personal information. There are no contact forms, intake forms, or data-entry fields anywhere on this site.

We do not use website analytics, advertising pixels, or tracking cookies, and no cookies are set when you visit.

When you book an appointment, the booking button takes you to our patient portal, which is operated by our practice-management provider on their own systems. Anything you enter there is collected by that system under its own terms and privacy policy, not by this website.

As with any website, our hosting provider keeps standard server logs recording the IP address and browser type of requests. Those logs are a normal part of operating a website. They are not used to build profiles of visitors, are not connected to patient records, and are not shared for advertising.

How We Use Your Information

Information you give the practice directly — by telephone, in our patient portal, or in person — is used to:

  • Schedule, confirm, and manage chiropractic appointments.
  • Respond to your questions and communicate with you about your care.
  • Provide treatment and maintain the clinical records the law requires us to keep.
  • Send administrative notices, and, only where you have agreed to receive them, health updates or practice news.

Protected Health Information and HIPAA

ChiroCare 360 is a healthcare provider and a covered entity under the Health Insurance Portability and Accountability Act (HIPAA).

Protected Health Information is collected and maintained in the practice’s patient-management and clinical-records system. It is not collected, transmitted, or stored through this website.

This policy describes the website. It is not our Notice of Privacy Practices, which is the separate HIPAA document describing how the practice uses and discloses your health information and what rights you have over it. Ask at the front desk or by phone for a copy.

Section 4

Sharing Your Information

We do not sell or rent your personal information to anyone.

The practice shares information with the service providers that support its operation, including our electronic health record and patient-scheduling provider, under agreements requiring them to protect it. Where those providers handle Protected Health Information they do so as business associates under HIPAA.

We may also disclose information where the law requires it, or to your health insurer in connection with a claim for your care.

Section 5

Data Security and Your Rights

We use reasonable administrative, technical, and physical safeguards to protect personal information the practice holds. No system can be guaranteed completely secure, and we do not claim otherwise.

You may ask to see the personal information we hold about you, ask us to correct it, or ask us to delete it, by contacting us with the details below. Your rights over your clinical records are additionally governed by HIPAA and Florida law, which in some cases require us to retain records even after a deletion request.

Section 6

Text Messaging (SMS)

If you give us your mobile number, our office may send you text messages about your care — appointment confirmations and reminders, and notices telling you that new forms are waiting in the patient portal. We do not send marketing or promotional text messages.

You give us your mobile number directly, on your new-patient intake paperwork, where our SMS consent form explains the program. This website does not collect phone numbers, and there is no text-message signup on this site.

Text messages are sent for us by our practice-management provider, acting on our behalf as our service provider.

We do not sell your mobile phone number. We do not share it with third parties or affiliates for their marketing or promotional purposes. Text-message opt-in and consent information is never shared with anyone.

Our text messages carry appointment and account information only. We do not send clinical details by text. If we need health information from you, the message will tell you to log in to the patient portal.

You can stop the messages at any time by replying STOP. Our Terms and Conditions set out the full text-message terms.
